Root & Sail Integrative Mental Health provides:

- Telehealth services for eligible patients in Pennsylvania

- Patient portal access through TherapyNotes
Provider Qualifications
Jacqueline Partee, DNP, CRNP, PMHNP-BC is a licensed Psychiatric Mental Health Nurse Practitioner providing services within her scope of practice and state licensing requirements.
Patient Responsibilities
Eligibility
To receive services, you must:
- Be located in Pennsylvania during telehealth appointments
- Provide accurate and complete information
- Be 18 years or older, or have parental/guardian consent if a minor
- Have the capacity to consent to treatment
Appointment Responsibilities
- Attend scheduled appointments or provide adequate notice for cancellations
- Ensure a private, secure environment for telehealth sessions
- Have reliable internet connection and appropriate technology
- Follow treatment recommendations and medication instructions
Payment and Financial Policies
Payment Terms
- Payment is due at the time of service
- We accept major credit cards and HSA/FSA cards
- Superbills are provided for insurance reimbursement
- Good faith estimates are provided upon request
Cancellation Policy
- 24-hour notice required for cancellations
- Late cancellations or no-shows may result in charges
- Emergency cancellations will be handled case-by-case
Telehealth Consent and Limitations
Technology Requirements
You are responsible for ensuring you have appropriate technology and internet connectivity for telehealth sessions. Technical difficulties may require rescheduling.
Limitations of Telehealth
- Not appropriate for psychiatric emergencies
- Limited ability to perform physical examinations
- Dependent on technology and internet connectivity
- May not be suitable for all conditions or individuals
Emergency Situations
IMPORTANT: Our services are not intended for emergency situations. If you are experiencing a mental health emergency:
- Call 911 for immediate danger

- Go to your nearest emergency room
- Contact your local crisis intervention services
Confidentiality and Privacy
We maintain strict confidentiality in accordance with:

Mandatory Reporting
Confidentiality has legal limits. We are required to report:
- Suspected child abuse or neglect
- Suspected elder abuse
- Imminent danger to yourself or others
